DAILY ATTENDANCE REGISTER - This form is essential and allows each teaching list, clinic or other activity to be recorded. There are 3 'sessions' per sheet. The sheets can be photocopied back-to-back.
CPD FORM - You are required to produce a summary form of continuing professional development at your RITA. It includes details of both external and internal points accrued.
DRIPEX Day Release In Programme Extra-contractual experience. This means that if you have set up a research project at one hospital, but in the meantime have rotated to another, you can take approx. a day a week to return to the previous hospital to complete your research. This all has to be properly supervised and counts as your research module. The rules are quite complex, and so you need to scrutinise the instructions that come with the form.
OUT-OF-PROGRAMME TRAINING/RESEARCH (OOPT/OOPR) This is the form and explanatory information produced by the RCA in July 2007. Prospective approval is required for any period of training outside the School to count towards CCT. The process involves the Deanery, RCA and PMetB.
RCA Registration form for new StR This is the form and explanatory information produced by the RCA in August 2007. It is the Registration form for all new ST in Anaesthesia - broadly speaking anyone in ST 1-3. You still need to complete this form even if you were registered as an SHO. Really, anyone in the ST run-through training programme who is not registered as a Specialist Registrar (SpR) needs to complete this.
STUDY LEAVE CLAIM FORM. This is the new (August 2007) form for claiming payment of study leave expenses after 1st August 2007. All London or KSS Anaesthesia SpRs and all ST3+ trainees should claim expenses for approved study leave taken after 1st August by completing this form and sending it to the SESA administrator (address on form). The study leave budget is currently administered by the Postgraduate Centre, Guy's and St Thomas' Trust. Study Leave should be approved locally as usual. This form is, then, for all LDN/091 or KSS/091 SpRs, and STs in year 3 or above who have attended a course/event/meeting after 1st August 2007.

CCT APPLICATION FORM - This is the form which is required by the Royal College of Anaesthetists to start the process of applying for CCT. Usually completed for the RITA G process and handed in to the Programme Director.
RITA Purple and Yellow forms - These are for School use only, but are the forms which trainees complete on the day of a RITA.
